Hi, Nice post, all acceptable. My opinion about two points:

9. portfolio: if you maintain confidentiality agreements you should not show the work.

10. online-order-form: is not possible if you don’t use a fixed price for all projects. I think it’s not right -for customers- use a fixed price for this service. I consider each project needs a personal quote. That is because the cost and time depends on:
- complexity of the layout(first page or homepage).
- complexity of additional pages(or new elements into main area, some elements can be reused from home or first pages).
- really special bulk orders.
